The Out of Hours Period is:

 

Monday to Friday:        6.30pm to 8.00am

Weekends:                    6.30pm Friday to 8.00am Monday

 

All Public and Bank Holidays


If you telephone the surgery during the Out of Hours Period your call will automatically be diverted to the Gwent Out of Hours Service.    Specially trained NHS staff will answer your call.  You will be offered advice on the telephone or be given an appointment to see a Doctor or nurse at a Primary Care Centre.   If the clinical situation requires a home visit you will be seen at home by the Doctor on call.  

  

This Out of Hours service is commissioned by Aneurin Bevan Local Health Board and any comments on the service should be directed to the Chief Executive.

 

The Wye Valley Practice participates in the "Informing Healthcare Programme" which links Monmouthshire pracices to the Gwent OOH service.  Under strict confidential/audited pathways some of your immediate past medical records, i.e. allergies, recent test results and medication lists, will be made available to the doctors working within the Gwent OOH service.  Your consent to view these records will be requested at the point of contact.  Should you wish to opt out of this programme please notify the  reception staff who will issue a Patient Choice Form for completion.  Should you require any further information please contact the Practice Manager

 

Many patients just want advice during the out of hours period, a starting point for this advice is to ring NHS Direct.

REGISTERING WITH THE PRACTICE

                                                                                               

 

Patients can register at the surgery of their choice, usually their most local surgery. One family member may register the whole family as long as the relevant documentation is completed.  This usually means handing in your signed NHS medical card but if this is unavailable then the surgery will ask for a form to be completed for each patient. On the form you will asked to say which Doctor you would prefer to see.  However you will be registering with the Practice rather than an individual GP.  You will be free to attend either surgery but you may be asked to attend a particular surgery to see a specific doctor or nurse, or to obtain an early appointment.


 

 

PATIENTS WITH PARTICULAR NEEDS

                                                                                               

 

Both surgeries are accessible to patients using a wheelchair, disabled parking and toilet facilities are available.  There is a "LOOP" system available for people with hearing difficulties.  We have access to special telephone facilities for patients who have hearing difficulties and patients who do not speak English.  If requested information leaflets can be made available in large print or Braille.

Does your Doctor know you are a carer? Ask your Doctor to make it clear on your notes and on the notes of the person you care for.

 
 
COMMENTS AND COMPLAINTS

                                                                                               

 

The Practice aims to provide a friendly and professional service at all times.  To ensure the highest quality of care the management team are keen to receive new ideas, comments or criticisms about the way the practice provides its services.  You may wish to pass on your thoughts via the Patient Paticipation Group.  There are seven patient representatives working on your behalf in this group.  They are:

There are occasions when patients are dissatisfied with the service they have received and in these circumstances the Practice operates a formal complaints procedure in accordance with the NHS complaints procedure. An information leaflet is available, from the Practice Manager, which offers guidance on the most effective way to make your complaint.  It is hoped that by following the guidance in this leaflet any concerns may be resolved at practice level to the satisfaction of all involved.

Tuesday, Thursday and Friday
8.30am - 1.00pm

Trellech

Monday, Wednesday and Friday 
8.30am - 1.00pm

Tuesday and Thursday
8:30am - 6:30pm

On Friday afternoons the surgeries will alternate between St Briavels (1st and 3rd) and Trellech (2nd and 4th).  For 5th Friday refer to notice.  They will be open for enquiries 1.00pm-6.30pm, and an open evening surgery 4.30pm-6.00pm
